SD43 Application Process
Follow these Steps:
To apply as a Program Partner with SD43-Afterschool Guide and offer affordable programs to support children and youth in collaboration with the SD43 Community Connections team:
Part 1 - SD43 Application and Review:
Step 1 - Complete the SD43 Partnership Application form here and email it to us.
Step 2 - Fill out and send in the Program Proposal form (also located under the "Program Provider" tab here)
Step 3 - SD43 will review your application, proposals, and references and may call you or schedule a meeting to discuss the program proposals, policies and guidelines. You'll need to complete and sign any additional agreements.
Part 2 - Other Required Information:
Step 1 - Once SD43 approves your proposal, please send in the following:
Provider Info form - to confirm your legal organizational or business status, insurance status etc. (also located under the "Program Provider" tab).
Instructor Info form - to confirm instructor info and CRC (Criminal Record Check) status for SD43 programs (also under the "Program Provider" tab).
Step 2 - Review the Instructor Guide, which has critical safety policy information and mandates from SD43. Make this part of your mandatory training for instructors/staff that would teach programs in SD43 (also located under the "Program Provider" tab).
Step 3 - Once completed, the approved program(s) will be Listed as a "vetted" SD43-Afterschool program and promoted to school partners in the upcoming season or term. We will forward you any questions or requests from schools.
Part 3 - DWUL Community Platform:
Step 1 - Create a "Provider" account on the DWUL platform (dwul.io) and set up your provider profile or watch the 60-second demo video.
Step 2 - Post your programs on DWUL when we forward you any requests from SD43 schools. You can watch the following demo for assistance:
Step 3 - Review the "Provider Guide" for other info guides about the DWUL platform tool (also located under the "Program Provider" tab).
You'll use your DWUL "Provider" account to post your programs for registration and manage communications with schools and student lists using the DWUL platform for SD43-Afterschool Guide partnered programs.
